Going into hospital can be a worrying time for anyone, but it can be even harder for someone with complex needs or a disability. Providing information about your cared-for person’s routine and specialist needs can help support both you and your cared-for person when they are in hospital.
University Hospitals Dorset (UHD) have a ‘This is Me – My Care Passport’. This can be completed by you and your cared-for person and taken into hospital with you. This will give staff important information they need to know about the person you care for such as their medications, their likes and dislikes and how they express their feelings etc.
